Location: Anna Baumer Community Center, Nature Way, Paducah, KY 42001

Details: Staff will be on-site to assist patients with scheduling appointments with a certified provider. Once booked, patients will see the provider at the event. Staff will then assist qualified patients with their state applications, and a notary will be available to notarize forms.

Cost: Patients pay the notary directly. De'Chenea Lovett is confirmed as our notary and will honor a $10 fee per notarization.
