Our patient drives and partners aim to keep patient costs low while ensuring you are supported from certification to card receipt, and beyond.

Eligibility
Gather your supporting documents for a permissible diagnosis:

Consultation
Visit a medical cannabis practitioner to obtain a written certificate. Providers are listed by name and specialty. Practices are not listed so check to ensure any practice you consider visiting has approved providers working for it. Any providers we partner with for patient drives are approved through the Office of Medical Cannabis.
Application
Once you receive a medical cannabis recommendation from and approved provider, patients must register for an account and access the Patient and Caregiver Registration Portal
“View Available Certifications” on the “Practitioner Written Certification Information” tab to retrieve your written certification.
Upload a digital copy of your signed and notarized signature page on the Documents tab. Signature pages can be downloaded from “Forms” at kymedcan@ky.gov.
Many banks will notarize for free for their customers
Print Your Card
Approved cardholders will be issued a digital registry ID card within thirty (30) days. Access your digital registry ID card through the portal from the Licenses tab. Download, save, and print a PDF copy by selecting Print Digital Card.
Take your card to a licensed medical cannabis dispensary.
